Learn how LG Health is changing and innovating to meet the requirements of healthcare reform:

Early in August 2014, Tracy faced a frightening transition following quadruple bypass surgery – it was time to go home.
Fortunately, Tracy was able to go to the home of a friend to recuperate. A few days later, she received a call from Toni Drumm, Medical Home Care Manager from her family practice, Twin Rose Family Medicine.
For years, John had been proactively managing a severe heart condition. Under the care of Dr. Roy Small, director of the Heart Failure Clinic, a program of The Heart Group of LG Health, Dr. Small ultimately recommended that John be evaluated for a heart transplant at Hospital of the University of Pennsylvania in Philadelphia.

Lancaster General Health is focused on three health priorities as part of its approach to Population Health Management in our community:

Lancaster General Health also seeks to increase patients' access to care by providing outpatient and specialty centers in convenient locations. Video In addition, collaborating with strategic partners enables LG Health to further improve patient services and outcomes.

University of Pennsylvania Health System (UPHS) of Philadelphia
After nearly 30 years of collaboration through the Penn Cancer Network, LG Health and Penn formed a strategic alliance in February 2014. In the upcoming year, we will discuss ways to strengthen our already successful collaborative efforts, while building new ones in patient care, research and education.
Centers for Medicare & Medicaid Services Bundled Payment Pilot Program
As a participant in this pilot program, LG Health will receive payment from Centers for Medicare & Medicaid Services (CMS) in one fixed amount for an "episode of care." That one payment will then need to be shared among the hospital and the doctors involved in the patient’s care – facilitating a more collaborative and cost-effective approach.
Accountable Care Organization
Lancaster General Health Community Care Collaborative, LLC, has been selected by CMS as the Accountable Care Organization (ACO) serving more than 20,000 Medicare beneficiaries in our community.
The Lancaster General Health Foundation advances the culture of philanthropy within the Lancaster General Health system. The Foundation also provides oversight of the fundraising activities for the Ann B. Barshinger Cancer Institute, the Heart & Vascular Institute, the Pennsylvania College of Health Sciences, Women & Babies Hospital, The Suzanne H. Arnold Center for Breast Health and other LG Health affiliates.
In 2014, the Foundation introduced the "Healthcare Hero" initiative, which gave patients the opportunity to honor an LG Health caregiver who made an impression on them. More than 142 caregivers were honored through this program, with patients making gifts to the Foundation in their names.
Gratitude expressed through philanthropy helps provide vital funds to improve patient care, to advance research, and to increase the margin of medical excellence available for our patients and their families. In fiscal year 2014, $4,828,967 was contributed to the LG Health system.
