How to Schedule Your COVID-19 Vaccine

We offer COVID-19 vaccines and booster shots at our family medicine and pediatric practices, as well as the Vaccine Clinic at the Suburban Pavilion. Vaccine availability varies depending on location.

Existing MyLGHealth users can schedule appointment online

  • Login to MyLGHealth and choose Schedule an Appointment from the main menu
  • Select: COVID-19 Vaccine
  • Select: Dose 1, 2 or 3/Booster
  • Answer questions, then select vaccine type, location, date and time 

If you DO NOT have a MyLGHealth account, you may create a MyLGHealth account then follow the steps above.

COVID-19 Vaccine Information

COVID-19 Vaccine for Children 5-11 Years Old


The CDC now recommends children ages 5-11 years old receive the Pfizer-BioNTech COVID-19 Vaccine.

  • The Pfizer COVID-19 Vaccine is the only approved vaccine for this age group.

COVID-19 Vaccine Boosters

Pfizer-BioNTech or Moderna

We are providing booster shots of the Pfizer-BioNTech and Moderna COVID-19 vaccines to:

  • Anyone 18 years and older who received their second dose of Pfizer or Moderna at least six months ago.
  • Schedule your appointment through MyLGHealth.


Janssen (J&J)

  • Patients 18 years and older should receive a booster shot at least 2 months after receiving their J&J COVID-19 Vaccine.
  • Schedule your appointment through MyLGHealth.
COVID-19 Vaccine First and Second Shots for Individuals Ages 12 and Older

Pfizer-BioNTech, Moderna and Janssen (J&J)

We currently offer Pfizer, Moderna and the Janssen (J&J) vaccines. Please note only the Pfizer vaccine is authorized for children ages 12-17.

Third Shots for Immunocompromised Patients

Pfizer-BioNTech or Moderna

The FDA authorized an additional dose of the Pfizer-BioNTech and Moderna COVID-19 vaccines for people whose immune systems are severely weakened. Please review the CDC guidelines.

  • Patients ages 65 and older who received their second dose of Pfizer or Moderna at least six months ago—and individuals between 18 and 64 years of age who meet additional criteria.
