Community Improvement


Introduction

Responding to the health needs of our communities, especially the most vulnerable among us, is central to LG Health/Penn Medicine’s mission to advance the health and well-being of the communities we serve. We constantly pursue a deep understanding of health needs and carry out a strategic approach to address those priority needs. Being the most comprehensive health care provider in Lancaster County is an honor we take very seriously, with intent to provide the highest quality care for those we serve, especially those in need.

We have been assessing and implementing strategies to address community health needs for more than a decade. In 1998, we initiated an organized process that is grounded in four key principles.

  • Community Collaboratives and Partnerships
  • Data driven needs assessment
  • Systematic approach to addressing the needs
  • Continual measurement and evaluation

The graphic below illustrates LG Health/Penn Medicine’s process for identifying community health priorities.

To further enhance the importance of community health improvement, our community-based Mission & Community Benefit Committee of the LG Health/Penn Medicine Board of Trustees was established. The charter of this committee is to assist our Trustees in fulfilling its oversight responsibility related to the health system’s mission and community benefit activities. These activities include but are not limited to taking necessary actions to increase the availability and access all members of the community to health and wellness services provided by LG Health/Penn Medicine and our partners, irrespective of their ability to pay.


This year, we once again collaborated with Wellspan Ephrata Community Hospital to conduct a Community Health Needs Assessment.  Please check back frequently for updates on our progress.
