Tracy’s Story: Life-saving Support From Her “Be Well Team”

Early in August 2014, Tracy faced a frightening transition following quadruple bypass surgery – it was time to go home.

Fortunately, Tracy was able to go to the home of a friend to recuperate. A few days later, she received a call from Toni Drumm, Medical Home Care Manager from her family practice, Twin Rose Family Medicine. Knowing that without health insurance, Tracy would not have the home healthcare that typically follows bypass surgery, Toni offered her help should Tracy need anything.

It wasn't long before Tracy took Toni up on her offer for medical advice; and thus began a special relationship. Toni continued to check in every few days – a lifeline that Tracy came to rely upon. "I love her. I really do," she says. Tracy's continued recuperation through the critical window of 30 days following surgery was a success, thanks to this vital connection to her care manager.

The two finally met in person at Tracy's first follow up at Twin Rose, where she has been under the longtime care of another member of her Be Well Team, Physician Assistant Yidrisca Vargas-Mirabal, or "Gigi." Tracy asked Gigi how she could acknowledge the great care she received from Toni. "She was genuine," Tracy explains. "It wasn't the amount of things she did; it was that she really meant it."

At Gigi’s suggestion, Tracy called LG Health's Care to Share line to convey her gratitude.
