About the Care Connections Program
We Provide
- Personal care created with and for the patient
- Timely visits with the health-care team and providers
- 24/7 access to the health-care team through our on-call service
- Tools to empower patients to set personal health-care goals and improve their overall health
Expectations for Patients
- Attend regularly scheduled office visits (usually every two weeks, based on needs) with provider
- Take an active role in setting and working on goals
- Participate in home visits (or agreed upon community location) with patient care navigator
Working with Our Team
Patients may work with the following Care Connections team members, depending on their unique goals:
- Patient care navigators
- Nurse case manager
- Clinical pharmacist
- Doctors and nurse practitioners
- Social workers
- Clinical service specialists
- Chaplain
New Patient Enrollment
If you have been identified as a new patient for Care Connections, you will have the opportunity to learn more about the program and be offered enrollment.
Below is the process for new patients:
- An enrollment date will be scheduled with you. A Patient Navigator will meet you at your home or another agreed-upon location in the community.
- Once enrolled, Care Connections will be your primary care physician (PCP) for 6-9 months. We will notify your home PCP that Care Connections will be providing your primary care and coordinating care while in the program.
- You will attend an office appointment with our team to review your medical history and prescriptions.
- On a regular basis, you will meet with members of our team, including medical providers, patient navigators, pharmacists, and social workers.
- After the program, you will return to your home PCP for future care and wellness support.
Appointment Information
For office visits, check in at the front desk. We will check your insurance information, address, and phone number at each visit.
Care Connections strives to provide easy access to appointments, many the same day. We encourage patients to contact our medical team at 717-544-2273 at any time, 24 hours a day, if they have symptoms or are feeling ill.
If you need to reschedule or cancel your appointment, please notify us as soon as possible, or at least 24 hours in advance.
Billing and Insurance
Care Connections currently accepts Medicare, Medicaid, and private insurance payments for our services. Before enrollment we will review your insurance to make sure services are covered.
At each office visit, co-pays may be collected when indicated. Lancaster General Health accepts Visa, MasterCard, and Discover.
